報告書に記載する名称（Name to be included in the report）
お申込時の返信メールに記載されている６桁の番号を入力下さい。（Please enter the 6-digit number provided in the reply email sent to you when you applied.）
セメント協会ホームページからダウンロードした試験成績報告票の様式から作成した報告票をExcelファイルでご提出下さい。（Please submit the report form created using the test results report form downloaded from the Cement Association website in an Excel file.） ファイル名は送信時に暗号化されるため、不連続の数値に置き換えられますのでご了承ください。
